After you attend a free information session, your next step is meeting with one of our bariatric surgery nurse clinicians. Before your first appointment, you will need to fill out and return the packet of information you receive at the information session. At the appointment, the nurse will walk you through our health assessment and help you submit a request for surgery to your insurance company for prior authorization.

Following is a list of questions to ask your insurance provider.
