At Park Nicollet, our bariatric surgery team is committed to providing patients with quality service in a caring environment. To consistently maintain high levels of service and care, we request current and former Park Nicollet bariatric surgery patients participate in two, quality-of-life surveys.

Objectives, benefits

These surveys help us understand how patients benefit from bariatric surgery, months and years after treatment. Feedback is used to improve the quality of life for all patients. In addition to weight loss, we track patients' overall health. Data is used to better support patients and shape our program to meet patients' evolving support, educational and physical needs. Your data also contributes to essential, industrywide medical tracking.

Two surveys

During the group information session, you will be asked to create a user name and password for accessing online surveys. When your user account has been activated, our staff will call to notify you and also to schedule your initial health assessment with a registered bariatric nurse clinician. We request you complete these surveys before your appointment.

• Quality of life 1 (SF36V2) – Tracks general-health quality of life
• Quality of life 2 (IWQOL LITE) – Tracks weight-loss quality of life

Time requirements

Quality of life is monitored in specific increments. Surveys are taken before surgery, every six months for two years after surgery, then annually. When you schedule follow-up appointments, we will remind you to take surveys, as required. Each survey takes about 5 to 10 minutes to complete.

Confidentiality, security

The information you provide in this survey is for Park Nicollet Health Services' internal use only and will not be shared outside Park Nicollet. The information will not be used for any marketing purposes. The way that you send the information to us is via a secure Web site. Once the information is received by us, it will be stored in a secured database.

Access to the database is limited to Park Nicollet Bariatric Surgery Center and data analysis staff, and controlled by a userID and password. You will be identified only by your userID, Park Nicollet member record number (MRN) or both.
