Description

Primary care physicians and clinicians diagnose and manage hundreds of different acute and chronic conditions. They need to be able to quickly access and respond to clinical issues and contributing factors including lifestyle, social, family and financial barriers that impact care planning. They also need systems and strategies for keeping people up-to-date with health maintenance and preventive screening tests. This requires working with a team of health care professionals and specialists daily.
