Type 2 Diabetes
This program is designed for adults diagnosed with type 2 diabetes who do not take insulin or have received little or no diabetes education. Participants will receive an introduction to type 2 diabetes, including self-care skills training. Topics include medications, blood glucose testing, meal planning, exercise and eating out. The program is four sessions over a period of six months. The first session is about three hours. Participants then attend a two-hour session conducted after two weeks; three and six month visits are one and a half hours each. Knowledge and skills grow with each session, along with the ability to monitor and evaluate diabetes control. Call 952-993-3393 for more information.

The cost of Type 2 Diabetes is covered by most health insurance plans. Some plans require per-visit co-pays. Your doctor must refer you to this program. Call your doctor or ask about a referral for diabetes education. You are welcome to bring a family member or friend. |
