Health care directive
Minnesota law lets you inform others of your wishes about the medical treatment you want to receive if you become unable to communicate.
A health care directive helps you do this. It is a written document that allows you to name a health care agent to make health care decisions for you, if you are unable to decide. You must be at least 18 years old to make a health care directive.
To make a health care directive
You are welcome to use the Honoring Choices Minnesota health care directive form. Other health care directive forms are accepted that meet the following legal requirements:

be in writing and dated

state your name
-
be signed by you or someone you authorize to sign for you, when you can understand and communicate your health care wishes

have your signature verified by a notary public or two witnesses

include the appointment of someone to make health care decisions for you or give instructions about your health care choices.
