Health care directive
Minnesota law lets you inform others of your wishes about the medical treatment you want to receive if you become unable to communicate.
A health care directive helps you do this. It is a written document that allows you to name someone to make health care decisions for you, if you are unable to decide. You must be at least 18 years old to make a health care directive.
To make a health care directive
You can get a health care directive form from our Patient Relations Department, 952-993-5541; your attorney; or the Minnesota Board on Aging, 800-882-6262. You do not need to use a form, but your health care directive must meet the following legal requirements to be legal:

be in writing and dated

state your name
-
be signed by you or someone you authorize to sign for you, when you can understand and communicate your health care wishes

have your signature verified by a notary public or two witnesses

include the appointment of someone to make health care decisions for you or give instructions about your health care choices.
