Patient responsibility
As a patient, you have the responsibility:

to make full and complete disclosure of your medical history and symptoms before and during the course of treatment

to furnish information necessary to determine your ability to pay for services provided

to cooperate fully with the people caring for you

to respect the rights of other patients

to understand your health problems to your satisfaction

to be honest and direct with the people caring for you; and

to notify your doctor about any unexpected changes in your health
