Community Health Needs Assessment

Park Nicollet Health Services initiated its first Community Health Needs Assessment (CHNA) beginning in 2011 and continuing into 2012. The CHNA is designed to reach broadly into the community to identify needs, gaps and barriers to health and health services.

Through a process of primary research, data analysis, validation and prioritization, the assessment process identified the following seven key themes of need:

  • Mental Health
  • Obesity
  • Seniors
  • Connecting Community Resources
  • Access and Affordability
  • Wellness
  • Culturally Sensitive Care

Three additional areas of need were also identified:

    • Teen and Young Adult Drug and Alcohol Abuse
    • Teen Pregnancy and Teen Parenting
    • Smoking Cessation

These identified areas of community health need will be addressed in Park Nicollet Health Services’ Implementation Strategy for years 2013 through 2015.
