Our diabetes education team and health psychologist deliver programs and services tailored to meet the needs of patients and their families. We help people live well with diabetes by providing psychosocial therapy and an education program that includes evidence-based curriculum. With an emphasis on positive goal-setting and motivation, our program helps close the gap between learning about and living well with diabetes. Patients 18 years and older can be seen with a written referral from their primary care provider. Education topics include: insulin initiation and adjustment, pumps, newly diagnosed and gestational diabetes. Learn more about diabetes support groups and classes.
Patient referrals
We know health care providers have limited time with their patients. International Diabetes Center is here to support providers and patients who:
- need diabetes education at diagnosis
- need to start insulin
- have wide fluctuations in blood glucose
- want greater control over their diabetes
- have lifestyle issues affecting glucose control
- need to update diabetes knowledge
- may be depressed or have other psychosocial issues
