Appointments are available with cardiologists, cardiac surgeons and other health professionals from 8 a.m. to 5 p.m., Monday through Friday at Park Nicollet Heart and Vascular Center in St. Louis Park, Minn., by calling 952-993-3246.

Appointments also are available with cardiologists at Park Nicollet Clinic locations in Burnsville, Chanhassen, Maple Grove and Shakopee.

What to bring to your appointment

When you come for an appointment, please bring along with you:

• your medical history
• list of medications you currently take
• name and address of the doctor who referred you
• insurance  card

You also may need to bring X-rays and previous medical records. Ask your doctor what personal medical information you need to bring to your appointment.

If other information or materials will be needed, it will be discussed with you when you schedule your appointment.
