Insurance

At your visit, you will be asked for your insurance information. We accept most major insurance plans, including HealthPartners, Patient Choice, Blue Cross and Blue Shield, Medica Choice, Medica Elect and SelectCare, Araz and PreferredOne, among others. If your insurance coverage includes Medicare or Medicaid, bring a copy of your membership card.

Many benefit plans require that you make a copayment at the time of your visit. You may pay by cash, check, Visa, MasterCard or American Express.
Depending on your benefit coverage, you may receive a monthly statement.
