Rehabilitation and fitness center appointments, Ground Floor
Appointments are available with cardiac rehabilitation staff from 7:30 a.m. to 4 p.m., Monday through Friday.
Services offered in the Rehabilitation and Fitness Center include the following.
Cardiac Rehabilitation Phase II
This is a supervised exercise and education program, including EKG monitoring, for people who have a heart condition or have had heart procedures such as open heart surgery, stenting or balloon angioplasty. All services are offered by registered nurses and physical therapists.
Doctor’s orders are needed for appointments. Usually the doctor will make the referral, and a cardiac rehabilitation staff member will call patients to arrange an appointment. This program is covered by insurance, so be sure to bring an insurance card.

Phase III Rehabilitation
This is a supervised exercise and education program open to anyone with a cardiac or pulmonary history, has cardiac risk factors or those who may benefit from a supervised exercise program. Registered nurses and physical therapists closely monitor participants as they exercise to ensure safety.
A doctor needs to sign off on your participation in this program. This can be done by seeing your doctor or by calling the Rehabilitation and Fitness Center and we will work with your doctor. Insurance does not cover Phase III Rehabilitation and there is a fee to participate. To register or for more information, call 952-993-2650. Find out more.

Pulmonary Rehabilitation
Respiratory therapists join physical therapists to provide outpatient pulmonary rehabilitation. This program works with people suffering from chronic obstructive pulmonary disease, lung cancer, lung resection and other pulmonary conditions, such as asthma, bronchitis or emphysema.
Professionals offer supervised exercise and education to help participants understand and manage their conditions. Sessions may be covered by insurance, so check with you insurance carrier. Call 952-993-7022 to register or for more information.

The cardiac rehabilitation program has been nationally accredited by the American Association of Cardiovascular and Pulmonary Rehabilitation. The certification recognizes the highest standards in exercise, education and counseling offered by cardiac rehabilitation professionals.
