Through Navigator, physicians or their staff can:

• arrange a specialty physician visit
• schedule outpatient tests or procedures

To help speed the referral process, please have the following information available for the Navigator representative:

• your name and clinic
• type of referral needed – physician visit, outpatient test or procedure
• department or specialty required

The department scheduling the patient service will need:

• patient information including name, date of birth, address, city, ZIP code and phone number
• insurance information including name of insured and name, address and telephone number of insurance company

Navigator representatives are available 8 a.m. to 5 p.m., Monday through Friday.
