At all locations, a doctor referral is required.
Medicare requires that patients must be under the care of a doctor.
Medicare requires the therapist to submit a plan of care to be reviewed and signed by the doctor every 30 days following the start of treatment.

Other requirements
Insurance plans vary with the individual patient’s contracts. We recommend that patients contact their insurance carrier for details of their plan.
Restrictions to provide simultaneous care occur in the following situations:

outpatient therapy and home-care
-
outpatient therapy and short-term stay at a nursing home
Exceptions are based on whether the site provides the type of care needed, such as wound care or detailed video swallow exams. If you have questions, please contact us.
All patients’ care plans are renewed at 30 days.
