Home-care

Rehabilitative home-care provides intermittent care in patients’ homes, the home of a family member or an assisted living facility. Services, including physical therapy, occupational therapy and speech pathology, maximize patients’ level of independence and minimize the effects of disability or illness. We work closely with the home-care nursing team to provide comprehensive care. Other services available include nurses, social workers and home health aides. We also work with the hospice team.
