Home-care
Rehabilitative home-care provides intermittent care in patients’ homes, the home of a family member or an assisted living facility. Services, including physical therapy, occupational therapy and speech pathology, maximize patients’ level of independence and minimize the effects of disability or illness. We work closely with the home-care nursing team to provide comprehensive care. Other services available include social workers and home health aides. We also work with Park Nicollet Methodist Hospital's hospice team.
To qualify for home-care, the patient must be homebound, under the care of a doctor and the services received must be part of a care plan established and reviewed by a doctor.
A patient does not need to be bedridden to be considered homebound. If the patient’s condition would require a considerable and taxing effort to leave the home, or if the individual should not leave the home except for medical reasons or for short durations, home-care is appropriate.
Home-care services must take place within 25 miles of Park Nicollet Methodist Hospital, and require a referral from the patient’s doctor, nursing home or Methodist hospital’s social services department.
