For general information or to schedule an appointment, call 952-993-3280 for Park Nicollet Clinic—St. Louis Park or 952-993-8700 for Park Nicollet Clinic—Burnsville. You do not need a doctor's referral, but you should check with your insurance carrier to ensure your visit will be covered under your policy.

Office hours

Our office hours are Monday through Friday, 8:30 a.m. to 5 p.m.  Some limited appointment times are available at 7:30 a.m. Call the numbers above for more information.

After-hours

For medical problems or emergencies on evenings, weekends or holidays, a doctor is on call at 952-993-3123.

Your appointment

• To prepare for your appointment, make a list of all prescription and over-the-counter medications you take. Include the dosage and frequency with which you take them. Please bring the list with you to your appointment.
• First-time office consultations usually require an hour.
• Please arrive 15 minutes early for registration.
• It is not necessary to fast for your first visit, unless you are instructed to do so by the doctor.
• If you are not a current Park Nicollet patient, please bring your outside medical records with you or ask your doctor to fax them to 952-993-1312.
• You will fill out a medication information form at every visit. It takes about five minutes.
