Medical records

When traveling outside the country, it is wise to bring your medical records. For convenient access to past medical records at Park Nicollet, sign up for Patient Online® if you have not already done so. Patient Online is a secured Web service that provides confidential access to parts of your medical record and allows you to e-visit with your doctor, nurse practitioner or physician's assistant.

If you are not yet a member of Patient Online, but are a patient of Park Nicollet, you may still request release of your medical records. If you are unable to locate or obtain your complete medical records, alert us during your consultation.
