My Areas of Practice
Diabetes education, Care Coordination (Adult & Pediatric), Advanced Care Planning (Advance Directives) facilitation
My role is to partner with patients and the primary care providers and staff at my clinic to provide additional support for patients with complex care needs and/or chronic conditions under the Health Care Home model of care. I do this by identifying & working to minimize barriers to care, providing health education, and being an advocate for patients when needed. I strongly value a holistic approach - meaning I take into consideration the unique physical, mental/emotional/spiritual, social/financial, and cultural influences for each patient. I love providing individualized education and resource guidance to help people be informed participants in their care, and I encourage patients and families to ask questions and seek clarity every step of the way.
I am also specially trained to provide education for adults with diabetes, including medications/insulin, diet, and physical activity. I enjoy working closely with patients to help them acheive a healthy, balanced approach to managing their diabetes.

I love spending time with my husband and our beagle, Rainer. We spend lots of time with family and friends and enjoy cooking, biking, and traveling together. I am an avid reader; historical fiction being my favorite genre. I am passionate about the many ways yoga can be helpful for a wide array of things and hope to start teaching prenatal yoga in the near future.
