My Areas of Practice

My role is to partner with patients/familes and the staff at the Eagan clinic to provide extra support to patients with complex and chronic health conditions.  This model of providing comprehensive, collaborative care is called Health Care Home.  My job is to identify & minimize barriers to care, provide health education and answer questions, connect patients to resources and specialty care, and serve as an advocate.  I strongly value a holistic approach, which I do by taking the time to learn about the individual circumstances of each patient.  I love providing personalized education to help people be informed participants in their care.  I whole heartedly encourage patients and families to ask lots of questions and seek clarity every step of the way.  Care Coordination services are available at no cost to patients who receive primary care at Park Nicollet.  

As a Certified Diabetes Educator I meet with adult patients with diabetes to help them understand the best ways and the options they have for caring for themselves.  This involves counsel about diet, physical activity, medications/insulin, and community resources. I really enjoy working closely with patients to help them acheive a healthy, balanced approach to managing their diabetes.

Personal

 

My husband and I spend lots of time with family and friends.  We enjoy cooking and traveling.  Our sweet puppy, Ruby, keeps life interesting and gets us outside for many dog park adventures.   I'm happiest in the sunshine with a good book on a beach or a dock on one of the many beautiful Minnesota lakes.  I'm passionate about the many ways yoga can be helpful for a wide array of things and hope to start teaching prenatal yoga in the near future.
