As a Care Coordinator for Park Nicollet Champlin my passion is to empower my patients and their family members to become advocates for themselves and their families. I work alongside with my clinicians in coordinating care with internal and external medical specialists, along with community resources, schools, and other outside agencies. I work with all ages from newborns to geriatrics.
I have worked in nursing for the past 18 years. In 2012 I moved into the role of care coordinator for pediatrics at the 3850 building. This position gave me an opportunity to use my public health nursing knowledge to assist my patients and their families. I coordinated care with the schools, medical specialist, community resources, along with infant/child mental health specialists. I helped parents find their voices. They learned how to be heard along with being advocates when it came to their children. The parents were able to work with the provider verses being told what they needed to do. I was an advocate for the patient and families.
I recently moved into the position at Champlin in September 2013. I have worked as a RN for over 18 years and this opportunity allows me to work the whole life span from infants to geriatrics. In my role not only am I an advocate to my patients and family members I also get to be an educator. I am able to work with chronic diseases and assist managing them such as asthma, diabetes, and high blood pressure.
Recently in 2013 the average age for a senior citizen in the US was 75 years old. I recently became a facilitator for Advance Care Planning and POLST. Honoring Choices MN is working to have patients and family members understand the importance of Health Care Agents and Health Care Directives. As parents get older and children are making decisions and caring for them they need assistance with understanding their roles and I am able to help educate and provide resources.
