Camp/sports physicals

Many camp and sports organizations accept physicals that have been done in the past two to three years. We suggest you check the physical information form supplied by the organization to determine how recent the physical needs to be.

If the physical was conducted recently

Most organizations require a signed form. Please bring your form to your clinic or mail it to your provider. Include a stamped return envelope and allow a one-week turnaround.

If you need to schedule a camp or sports physical

Please call your clinic for an appointment. Try to schedule your visit several weeks in advance of when you need the completed form.
