Park Nicollet Health Services understands that vaccine safety is an ongoing concern for parents. The following information is to help parents and caregivers understand some of the common issues and questions surrounding this topic.

Obtaining a copy

If you would like a copy of your or your child's immunization history, please call your clinic and ask for the medical records department. 

You can also print your child’s immunization records by signing up for a proxy account on Patient Online. Proxy memberships are available on behalf of another adult and children ages 0 to 10. Due to privacy laws, proxy memberships are not available for minors ages 11 to 17.
