Special needs

Handicap motor vehicle stickers

First, obtain a Disability Parking Certification Application by contacting the Minnesota Department of Public Safety, Driver and Vehicle Services at 651-296-6911 or obtain the form via the Internet (Adobe Acrobat Reader is needed to read this form.)

Second, mail or bring your form to your clinic to obtain your provider's signature. If you have not seen your provider in the past six months, you will need to make an appointment to be seen.

Hearing-impaired

All clinic locations call 1-800-657-3775 (Minnesota Relay Service).

Language interpretation

We will make every effort to accommodate patients who require language interpretations or have other special needs due to visual or hearing impairments.

Please notify your clinic staff before your visit if you have special needs.

Medical equipment

If you need a prescription for insurance purposes, please call your provider.
