Patient referrals

We know health care providers have limited time with their patients. International Diabetes Center is here to support providers and patients who:

• 
• need diabetes education at diagnosis
• need to start insulin
• have wide fluctuations in blood glucose
• want greater control over their diabetes
• have lifestyle issues affecting glucose control
• need to update diabetes knowledge
• may be depressed or have other psychosocial issues
