Diabetes Connections

Diabetes Connections® is our full package of consulting services. In this three-phase consulting program, we partner with you to assess and implement strategies needed to achieve high-quality diabetes care and education. Diabetes Connections is facilitated by experienced consultants and engaging speakers who provide current, evidence-based content.

  1. Organizational assessment. Our consultants engage your staff, promoting relationships that contribute to success. Through discussion of what is working and what needs to be addressed, plus data analysis, we provide a comprehensive report that captures your vision with key recommendations and a clear action plan to get you there.
  2. Evidence-based training

    • Staged Diabetes Management™ training. Like no other CME program, “Staged Diabetes Management” (SDM) is designed for the entire care team. SDM training translates the latest research into practice to treat diabetes and its associated disorders. Through clinically proven guidelines and algorithms, skill building and sharing of strategies to integrate diabetes care and education, your organization develops a systematic approach to treating diabetes. Your staff will leave feeling energized as they gain recognition and respect for the value each care team member brings to improving quality and patient outcomes.
    • BASICS training. BASICS is our evidence-based patient and professional curriculum. Designed for diabetes educators, this training helps your staff gain new insights through tips and tools for working with patients and primary care to improve outcomes.
    • Continuing medical education follow-up module options. These consist of “Moving the Needle in Primary Care” or “Hypertension and Diabetes.” Based on your assessment at six months after the SDM training, you can select either program. Skilled IDC trainers present a two-hour CME program that provides the latest in research, using tools and algorithms that are applied in a case-study format.
  3. Follow-up: keeping the momentum. The key to success is continued program implementation. Monthly phone consultations, regular audits of care and a six-month site visit and CME program provide the focus, tools and support needed to meet and surpass national benchmarks.
