STAGED DIABETES MANAGEMENTSM Program

STAGED DIABETES MANAGEMENTSM, also identified as SDMSM, provides expertise and facilitation skills to help primary care providers and other staff lower costs and reduce diabetes-related complications. Participating health systems have reported:
  • blood glucose improvements in patients treated by nurse case managers, as shown by a 1.7 percent reduction in glycosylated hemoglobin A1C
  • lipid screening improvement from 77.6 percent to 91.1 percent in SDMSM patients

Because SDMSM exceeds standards recommended by the American Diabetes Association, many training sites have achieved Diabetes Provider Recognition from the National Committee on Quality Assurance. Such recognition ensures improved care quality and diabetes education reimbursement.
