Implementing STAGED DIABETES MANAGEMENTSM

Phase 1: Program introduction

Trained presenters introduce STAGED DIABETES MANAGEMENTSM (SDM)SM to health professionals, administrators, patients and others. Data from SDM studies in a variety of settings are presented, along with an overview of SDMSM materials. A diabetes team and "A" champion are identified to facilitate site preparation and the program adoption process.

The health care organization undergoes a needs assessment and chart audit to collect data and establish a baseline of diabetes care. Complete, accurate baseline data allows the effect of SDMSM on diabetes care and outcomes to be measured. We provide audit tools and assistance as needed.

Phase 2: Training and customization

Facilitators guide a consensus-building process to customize SDMSM DecisionPaths, a roadmap to help guide decision-making. Customization is based on community practices and resources. By using case studies, attendees experience application of the guidelines in practice. Using the baseline data, the diabetes team sets priorities, identifies outcome goals and develops an action plan.

Phase 3: Implementation and consultation

Evaluation tools and documentation systems for collecting ongoing data are established. The team ensures that the customized SDMSM DecisionPaths is applied in practice by the entire community. Outcomes data are collected, evaluated and compared to the baseline data. Regular telephone consultations facilitate progress toward achieving the implementation plan and clinical outcome data goals.
