Care schedule
A diabetes care schedule incorporates what you need to do daily, monthly, quarterly or yearly to care for your health. This includes following a food and activity plan, testing blood glucose, taking your medication and keeping a record of blood tests and insulin or medication doses.
This also means visiting your doctor or other health care provider about every three to four months, having your hemoglobin A1C tested, checking cholesterol and blood pressure, screening micralbumin and more.
To keep track of your diabetes care visits and tests, use the
diabetes care schedule, which shows all the tests and exams you can expect to have and the target results for each. Not all targets are ideal for all people, so talk to your health care provider about whether personalizing the schedule makes sense for you.
