As a care manager, I facilitate care coordination between referring providers and the Melrose Institute care team. I also serve as the contact for patients and families by answering questions; listening to concerns and comments; solidifying the treatment plan with care team providers, patients and families; and ensuring that we meet their needs.

Referrals from primary care doctors are recommended, but not required.

If your patient decides to receive treatment from us, they will be assigned a care team. This team includes a care manager, medical doctor, dietitian and therapist, who will work with them throughout treatment. The care manager will be your main point of contact, and will provide regular updates on your patient’s recovery progress.
