Patient survey to improve patient outcomes and care

Our patients deserve the highest quality of care. We want to know if we provided the best care possible, and how our patients are doing, during and after their treatment.

Every patient that completes an initial assessment, then receives treatment at Melrose Institute, is asked to complete an outcomes test three months, six months, one year, two years and three years after their assessment. Patients are asked about depression, self-esteem, exercise, anxiety, body image and many other eating disorder aspects to get a comprehensive picture of their progress.

Patients who fill out the survey are added to a drawing for a $150 Target gift card. Drawings are every three months, so each patient has a chance to win five times if they fill out all their testing.

This outcomes data will be used to improve treatment at Melrose Institute.

Take the survey.
