At this year's Paul M. Silverstein, MD, Community Service Awards, help us recognize those who have increased public awareness, improved services or advanced research of Parkinson's disease and related movement disorders in the Upper Midwest. You also can participate in the new "Grant a Wish" fundraiser for the Club CREATE day program. This is in lieu of a silent auction, and gives you opportunity to purchase items for our day program. In return, we will enter you in a drawing for a gift certificate for a food or entertainment venue as thanks for granting this wish.

Tickets: $125 per person (65 percent tax-deductible)
