2006 annual summary
Building healthy communities
Park Nicollet Foundation’s mission is to build healthy communities by responding to community health needs, and to foster and support programs and services that enable Park Nicollet Health Services to serve those needs.
In 2006, the Foundation distributed $2 million through 189 grants. Some of the highlights include:

Opened in 2006, Diamondhead Community Clinic provides primary care, mental health and personal health services for Burnsville School District 191 youth – infancy through high school. Services are provided free of charge for those without health insurance.

Healthy Choices project supports a Native American diabetes prevention program for students grades 7-12 and provides screening and case management, health education classes and an expanded exercise program.

Seed of Hope provides mental health counseling and services to families who cannot afford it.
Building relationships is a key component to Park Nicollet Foundation’s success. Our partnerships include municipalities, school districts, faith communities, businesses, nonprofit service organizations, local clinics and neighborhoods in more than 35 communities.

Partnering with school districts, donors and Methodist Hospital Hospice, the Foundation was able to provide grief-and-loss support resources to students in nine school districts through the “Growing Through Grief” program.
