PN scores 100%

Park Nicollet (PN) is one of two health care systems to achieve 100% of benchmark performance measures in an important Medicare pilot project that is receiving national attention.

In data released on August 17th, the Centers for Medicare Services (CMS) announced that Park Nicollet achieved all of its 32 benchmark measures in the Physician Group Practice (PGP) demonstration project. PGP targets five areas where health systems design special programs to reduce hospital readmissions and provide additional savings to Medicare: congestive heart failure, coronary artery disease, diabetes, hypertension and breast and colorectal cancer screenings.
