Bring photo ID to patient check in.Bring photo ID when you come to the clinic

Starting Friday, May 1, patients at all Park Nicollet clinics and Methodist Hospital will be asked to show photo ID when they check in for treatment or service. This new process will help safeguard against identity theft and is required under the Fair and Accurate Credit Transaction (FACT) Act of 2003. Like asking for an insurance card, asking for photo ID will help ensure we have the correct patient information – and protect your identity.
