Illness-causing difficulty with ADLs or appropriate follow-through with health recommendations because of emotions (depressed, fearful, anxious …), social (family, work) or physical problem (mobility, eating …)

• Hennepin County Human Services and Public Health
  Services include health promotion and assessment clinics, health education, WIC, immunizations, environmental health, mental health, chemical dependency, child protection, adoption and more.
• Park Nicollet Methodist Hospital’s home care services 
  Offers help with everything from skilled nursing care to taking care of your basic health needs.
• Park Nicollet Rehabilitation Services
  Offers a comprehensive range of occupational, physical and speech therapy services.
• Senior Community Services 
  Nonprofit human services organization offering a range of programs and services aimed at older adults, including Frail Elders Support Services, HOME (Household and Outside Maintenance for the Elderly), caregiver support and transportation.
• Volunteers of America of Minnesota 
  Provides help with problems and behaviors associated with depression, anxiety, changes in sleeping or eating habits, confusion, thought disorders and other issues that cause distress, put a person at risk or reduce the ability to function. Services provided in homes and in community locations in Hennepin, Ramsey and Anoka counties.
