Medical history

Keeping good medical records is an important part of being an active health care consumer. Many health problems run in families. Other health problems are easier to anticipate when you and your doctor have a complete picture of your personal medical history. That’s why it’s important to keep up-to-date records of your family and personal medical history. 

Medical History Worksheets, Preventive Examination and Immunization Records

Make copies of the “Family Medical History” worksheet, “Your Medical History” worksheet, and “Preventive Examination and Immunization Record,” and complete one for yourself and for each member of your family. Keep them and a completed “Medical Resources” worksheet with your medical records, and update them as necessary.
