Adverse health events reporting: Minnesota (Minnesota Department of Health): Minnesota law requires that hospitals disclose when any of 27 medical errors occur. This site includes the 27 types of incidents that are tracked and publicly reported including wrong-site surgery; retention of a foreign object in a patient after surgery; and death or serious disability associated with medication error.
