Medicare Physician Group Practice Demonstration
Park Nicollet Health Services recently was selected to participate in a three-year research study for the Centers for Medicare and Medicaid Services (CMS). The study is called the Physician Group Practice Demonstration (PGP).
As part of this study, patients with fee-for-service Medicare coverage will be eligible for some services not previously covered by Medicare. These services will be free. You will continue to get your regular Medicare-covered services, and you can still see your current doctor and any other doctor you choose. You do not need to take any action. Medicare HMO patients are not eligible for these services.
We are one of 10 care systems nationally participating to develop preventive care and chronic disease management programs that demonstrate improved patient care and service. PGP rewards doctors for giving high-quality health care. Park Nicollet will only receive performance payments if we improve the quality of care you receive. Our expected savings will be generated by keeping you healthy and preventing the need for hospital admission.
Our initial focus will be on chronic disease management programs for patients with congestive heart failure, coronary artery disease and diabetes. Please check back for more information as this program develops.
Park Nicollet will share information with Medicare about your participation in these services as a way to measure each service's success. Your information will remain secure and confidential.
