Obtaining a copy of immunization records

Call your clinic and ask for the medical records department to request your or your child’s immunization history. If you and your family are Park Nicollet patients and you have a Patient Online account, you can:

• print your immunization record directly
• print your child’s immunization record directly, if you have signed up for a proxy account. Proxy memberships are available on behalf of another adult and children from infants to age 10. Due to privacy laws, proxy memberships are not available for minor children, ages 11 to 17.
