Transferring a prescription

As a convenience and to save time, we recommend you request a transfer by telephone. Or, you can request a transfer at the counter of any of our clinic-based pharmacies.
 
Steps to transferring a prescription from your current pharmacy to Pharmacy @ Park Nicollet

Step 1: Choose one of our clinic-based pharmacies.
Step 2: When calling the pharmacy of your choice, you will be asked to choose from several service options. You do not need to listen to all selections, simply press “0” at that time to speak directly with our pharmacy staff.
Step 3: You will be asked to provide or verify the following information, whether you have opted to transfer a prescription by telephone or in person, so please have this available:
  • patient name and prescription number of the prescription you want transferred
  • name and telephone number of the pharmacy where you currently have prescriptions filled
  • drug insurance information, including your member number and the name of your insurance provider
  • name, address, telephone number, date of birth and allergy information
Step 4: Your Park Nicollet pharmacist will complete the transfer of your prescription on your behalf. Depending on the nature of your prescription, please allow for appropriate process time. Our pharmacist can provide more details.
