When your hospital stay is over, you might not be quite ready to return home. That’s where the After Hospital Care Network comes in. We connect you with transitional care options that give you the support you need to fully heal.

What is transitional care?

Transitional care is a step between the hospital and your home designed to help you transition back into your daily life. In transitional care, you’ll get help with daily activities, medical assistance and physical, occupational or speech therapy as needed.

There are two main types of transitional care locations:

Transitional Care Unit (TCU)

If you’re still recovering from an illness, injury or surgery, you might choose a TCU. A TCU is best for patients who need short-term care. Here, patients get recovery assistance to help them heal safely and rehabilitation therapies to regain strength and independence. Patients usually spend fewer than 21 days in a TCU.

Skilled Nursing Facility (SNF)

A SNF is the best fit for patients who need long-term care. At SNFs, nurses are on-call 24/7 to provide expert medical care and assistance with anything from getting dressed to taking your medicine. SNFs are a good choice for recovering patients that still need round-the-clock care.

We have a list of preferred after-hospital care network locations. These locations are throughout the Twin Cities and provide high-quality care from experienced doctors.

Do I have to choose from one of the preferred locations?

No. You should choose the location that works best for you and your family. We’ll provide you with a list of area SNFs and TCUs for you to choose from during the discharge process.

There are benefits to choosing from the network. Staff at our preferred locations share access to patient records and communicate closely with Methodist Hospital to ensure coordinated patient care. Our doctors and nurse practitioners also spend more time on-site at the network locations than other SNFs and TCUs in the area.
