Our team of geriatricians, nurse practitioners, social workers and other specialists work together to make getting the care you need as easy as possible. We see patients at assisted living facilities, long-term care facilities, transitional care unit or your own home in Minnesota and western Wisconsin. We also offer video visits for many different health concerns, so you can talk to a doctor face-to-face from the comfort of your couch.

When you do need to visit the clinic, we have you covered. HealthPartners and Park Nicollet make up the largest care group in Minnesota. With over 90 clinics in the Twin Cities and western Wisconsin, the care you need is always nearby.

Community senior care

Our Community Senior Care nurse practitioner and physician teams provide care to patients at assisted living, long-term care, and transitional care facilities throughout the Twin Cities and western Wisconsin. Our teams are also available to provide care in your home in coordination with your primary care clinic provider. You don’t need to worry about scheduling rides to and from your appointment, going out in bad weather or wasting time sitting in waiting rooms. We bring health care to you to help manage your chronic and minor acute health concerns.

Living well as you age means that regular check-ups more important than ever. To help keep your health on track, we provide primary care visits, geriatric assessments and emotional support.

Primary care for seniors

Our primary care doctors are trained to diagnose and treat hundreds of health conditions. We help with everything from keeping your immunizations up to date and regular wellness check-ups to treating illnesses.

We’ve designed our clinics to provide the best care and convenience for our patients. Many of our clinics also include lab, radiology and other onsite conveniences so you can get everything you need without making multiple stops.

Emotional support

Many people struggle with feelings of loneliness, anxiety and depression as they grow older. A change in life or abilities can feel isolating and frustrating. Your primary care doctor can connect you with one of our mental health experts to help you get the support you need.

Orthopedics and sports medicine

Finding an activity you enjoy as you grow older is a good way to retain strength, flexibility and balance. If you slip while on a walk or feel a twinge in your knee while dancing, our team is here to help. Our orthopedics team is here to treat everything from hip or knee pain to broken bones. In fact, Regions Hospital is one of five hospitals in the country certified as a geriatric fracture care program by the International Geriatric Fracture Society. That’s the highest certification possible.

Help with caring for an older relative or friend

Caring for an older relative or friend, while often rewarding, can also have its challenges. We can give you the support you need, from extra help at home to memory care to helping you navigate changes in behavior.

Home care

If your loved one needs extra care at home, our nurses, social workers, home health aides and physical, occupational and speech therapists come to you to provide personal care. We provide these services at houses, apartments, group homes, assisted living facilities and residential housing throughout Minnesota and western Wisconsin. If you think that you need home care, a nurse or therapist will be at your home within 48 hours of your application to discuss your needs. Please note that an order is needed for home care.

Memory care

When someone you love has memory or cognitive disorders, each day can have new challenges. It could be dementia, Alzheimer’s disease or age-related memory loss. We can help determine what is causing the memory loss and be a guide for you. Our memory care team and research professionals provide patients, family members and caregivers with everything they need to live their best lives. We’ll be with you every step of the way, offering therapies based on the latest research.
