Creating a Care Plan
When you enroll in hospice, you work closely with your hospice team to create a care plan unique to your wishes and needs. A care plan helps to communicate how you wish to be cared for now and in the time ahead.
Determining the physical, emotional and spiritual support you want helps guide the hospice services you receive. A care plan also can provide relief for your family because they know you’re being cared for as you want.
A care plan helps you, your family and the hospice team know what’s important to you. Knowing your specific wishes can help ease the burden and potential conflicts when decisions have to be made. Our goal is to help you and your family experience hospice with courage and a sense of togetherness.
Choosing care services
It often can be difficult for doctors to know how your illness will prgross. Creating a care plan helps you to:
